Yesterday while doing an extensive inventory in my workshop I had this occur many times;

IPad 2: iOS 5.01

Create item entry, take item photo (flip iPad to portrait for photo) flip iPad back to landscape to set on workbench and finish data entry. As soon as I flipped back to landscape MyStuf2 crashed back to my home screen.
Remained running but then when I went back in I had to navigate back to the entry and the item picture was not there. Had to take it again, this time hitting save before flipping back to landscape.

(Note on orientation, when going from landscape to portrait to take the pic I would pick up the iPad and rotate it to portrait while moving the iPad from the workbench to ne'er the floor where I had layed out my power tools for each picture then after taking the picture moving the iPad back to the workbench and into landscape mode. Probably a lot going on there with the accelerometer.)

This happened several times on different items.

I learned eventually to save the entry then hit "edit" before I took the photo. That averted the issue and it didn't happen after I started doing that.

I tried following your steps but could not recreate the problem. But to be honest your list of steps aren't detailed enough to be sure I'm doing the same thing you are. At what point do you tap "Add Image"? At what point do you tap "Take Photo"? At what point do you tap the shutter button on the camera window? And at what point do you tap the "Use" button after taking the photo?

At what point do you tap "Add Image"? After I have created a new entry while iPad is sitting on table in landscape mode.
At what point do you tap "Take Photo"? after all data points are added to entry but before picking iPad up from table.
At what point do you tap the shutter button on the camera window? After picking up iPad and rotating to portrait mode to take picture of tool sitting on the mat on the floor.
And at what point do you tap the "Use" button after taking the photo? Never got there, crashed first. After hitting shutter button I moved the iPad back to the table rotating into landscape mode at which point it crashed back to home screen.

Thanks for the logs. I am still unable to reproduce the crash despite your detailed instructions. But I will keep trying and combined with the crash logs hopefully I can find and fix the issue.
